It is our policy not to collect personal information about you when you visit our website unless you specifically choose to make such information available to us. If you choose to share personal information by sending a message or filling out an electronic form, please be aware that under Florida law email addresses are public records. We must save it for a designated period of time to comply with the State of Florida's archiving policies. The information will not be disclosed to third parties or other government agencies unless required by state or federal law. 2. Make check/money order payable to Unemployment Compensation Trust Fund
3. Mail check/money order and completed coupon to Benefit Payment Control, P. O. Drawer 5050, Tallahassee, Fl 32314-5050.
